If you would like your child to attend, please indicate on the form below. The closing date for requests is 9am on Thursday 3rd of March.

Please note due to high demand, any requests received after this date WILL NOT be able to be accepted.

Please note: Due to limited staffing and high demand, the days allocated to individuals for the Autumn Program may be significantly reduced.

If you have an urgent need, or require any specific days please let us know in the comments section at the end of this form and we will try our best to accommodate you.
