OASIS materials returns
This form will be used to generate Fed Ex return labels for your OASIS materials.
Sorry for so many words--but there is a lot of info.

• Please be sure the information that you provide is complete and accurate.

• Please recognize that due to current staffing and workload, we can only create labels once. They expire quickly, so they MUST be printed and shipped within the week they are created.  If you miss that window, you will be responsible for the cost of returning materials to OASIS via your method of choice. USPS media mail is always the cheapest option.

• You will receive the labels in an email directly from Fed Ex--I will email you once labels are created, so please watch your inbox and check your junk mail if they don't show up in a timely manner. Print label, attach to boxes, and deliver them to your nearest FedEx pickup location--please do not schedule home pick ups as the cost is astronomical and threatens our ability to use this service.

• All items must be returned by the due date of June 16--please do not mail materials in the summer as school is not open and things tend to get lost. We do not have time or staff to process returns in the fall, so please get items returned before school ends.

• Please check your library account for a list of items checked out to you. If you click on the title, the components are listed so you can make sure you have all pieces. To avoid confusion, please pack sets together in the same box. Fall materials cannot be ordered or shipped until your library account is clear.

• If you are using materials again next year for another student, please renew items on a renewal form (below) and submit to your teacher. Please bear in mind the limitations on renewals (see renewal form).

• Lastly, please note that we will not be providing Fed Ex labels in the fall for returns. Fall is too frantic with material orders and shipping. Returns will not be processed until the materials rush is over, so to insure timely ordering of new materials, please return by the end of the school year.

Please confirm that you have checked that all sets are complete (including tubs for MBTP/Blackbird sets). If you need a list of components please check your library account. Every set has a list of components in the description if you click on the title. Sets cannot be checked in until they are complete.
