THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS!This Release and Waiver of Liability executed on the date listed below by Participant address and contact information on page one of this document in favor of “Regulators ,Bishop Fenwick High School, GAC Fitness” Training LLC and “Regulators ,Bishop Fenwick High School, GAC Fitness” Gym LLC, doing business as “Regulators ,Bishop Fenwick High School, GAC Fitness” Gym and Sports Training henceforth known as “”Regulators, “Regulators ,Bishop Fenwick High School, GAC Fitness” with offices at 4343 South Dixie Highway, Middletown Ohio 45005, and their directors, officers, employees, and agents.1. Acknowledgement of Risk. Participant understands and acknowledges that all exercises, including the use of weights and use of any and all machinery, equipment, and apparatus designed for exercise shall be at the members sole risk. The Participant also understands and acknowledges that all programs and classes offered by and through “Regulators, ,Bishop Fenwick High School, GAC Fitness”, and that fitness classes, and even self monitored activities in which Participant desires to engage (the “Activities”) while on the premises of “Regulators, ,Bishop Fenwick High School, GAC Fitness” entail certain risks. “Regulators ,Bishop Fenwick High School, GAC Fitness” urges you and all members to obtain a Physical examination from a doctor before using any exercises equipment, machinery, or participating in any exercise or training class. Participant acknowledges that while the staff of “Regulators, ,Bishop Fenwick High School, GAC Fitness”, have been trained to protect physical safety, participation is purely voluntary.2. Assumption of Risk. Participant understands the agreement to use or selection of programs, classes, trainings,methods, and types of equipment shall be the participant's sole responsibility. Activities may involve indoor or outdoor activities and or training, physical exertion, use of mechanical equipment, exposure to hazardous conditions, or other circumstances that may be hazardous. Participant hereby expressly and specifically assumes the risk of injury or harm in the Activities, and releases “Regulators, ,Bishop Fenwick High School, GAC Fitness”, from all liability for injury, illness, death, or property damage resulting from the Activities.3. Release and Waiver. Participant freely, voluntarily, and without duress, elects to participate in spite of the associated risk, and by signing this Release, knowingly and voluntarily assumes all responsibility for participation. Participant does hereby release and forever discharge and hold harmless “Regulators, ,Bishop Fenwick High School, GAC Fitness”, and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, that arise or may hereafter arise from Participant Activities with “Regulators, ,Bishop Fenwick High School, GAC Fitness”. Participant understands this releases “Regulators, ,Bishop Fenwick High School, GAC Fitness” from any liability or claim the volunteer may have against “Regulators, ,Bishop Fenwick High School, GAC Fitness” with respect to any bodily injury, psychological or emotional injury,personal injury, illness, death or property damage that may result from the Activities, whether caused by the negligence of “Regulators, ,Bishop Fenwick High School, GAC Fitness” or its officers, directors, employees or agents, or otherwise. Participant also understands “Regulators, ,Bishop Fenwick High School, GAC Fitness”, does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury, illness.4. Medical Treatment. Participant does hereby release and forever discharge “Regulators, ,Bishop Fenwick High School, GAC Fitness”, from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with the Activities.5. Media Release. Participant does hereby grant and convey unto Regulators, “Regulators ,Bishop Fenwick High School, GAC Fitness” all right, title, and interest in any and all photographic images and video or audio recordings made by, or on behalf of “Regulators, ,Bishop Fenwick High School, GAC Fitness” during the Activities and while on the premises of “Regulators, ,Bishop Fenwick High School, GAC Fitness”, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. 6. Other. Participant expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Ohio, and that this Release shall be governed by and interpreted in accordance with the laws of the State of Ohio. Participant also agrees that in the event any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.I. the signed below, the undersigned, on behalf of myself, my heirs and next of kin, personal representatives, agents, insurers, successors and assigns (all hereinafter “Releasors”) hereby FOREVER RELEASE, DISCHARGE AND COVENANT NOT TO SUE THE UNITED STATES WRESTLING ASSOCIATION, INC., its affiliated clubs, administrators, agents, directors, officers, state organizations, members, committees, volunteers, all employees of US Wrestling, and any and all participants, officials, referees, coaches, host clubs, sponsoring agencies, sponsors, advertisers, local organizing committees (and if applicable, owners, lessors and operators of premises used to conduct any US Wrestling sanctioned event, meet, practice or activity (all hereinafter “Releasors”) from any and all liabilities, claims, demands, causes of action or losses of any kind or nature, past, present, or future, direct or consequential that I may hereafter have for PERSONAL INJURY, PERMANENT, TEMPORARY, TOTAL OR PARTIAL DISABILITY, DISFIGUREMENT, PARALYSIS AND ANY OTHER LOSSES OR DAMAGES TO PERSON OR PROPERTY OR DEATH, arising out of participation in, attendance at or traveling to and from any USA Wrestling sanctioned event or activity including, but not limited LOSSES CAUSED BY [PASSIVE OR ACTIVE NEGLIGENCE OF THE RELEASES, or hidden, latent or obvious defects in the facilities or equipment used. 2. Releasor understands and acknowledges that USA Wrestling activities and the sport of wrestling in general have inherent dangers that no amount of care, caution, training, instruction, supervision or expertise can eliminate. RELEASOR EXPRESSLY AND VOLUNTARILY ASSUMES ALL RISK OF PERSONAL INJURY, PERMANENTLY, TEMPORARY, TOTAL OR PARTIAL DISABILITY, DISFIGUREMENT, PARALYSIS AND ANY OTHER LOSSES OR DAMAGES TO PERSON OR PROPERTY OF PASSIVE OR ACTIVE NEGLIGENCE OF THE RELEASEES, or hidden, latent or obvious defects in the facilities or equipment used. 3. Releasor acknowledges and fully understands that each participant in any USA Wrestling sanctioned event, meet, practice or activity, including Releasor, will be engaging in activities that involve risk of serious injury, including permanent, temporary, total or partial disability, disfigurement, paralysis and any other losses to person or property including death, and that severe social and economic losses may result not only from Releasor's own actions or negligence, but from the actions, inactions or negligence of others notwithstanding the rules of play or the condition of premises or of any equipment used. Further Releasor acknowledges and fully understands that there may be other associated risks with such activities which are not known or not reasonably foreseeable at this time. LIABILITY RELEASE
I/We hereby understand and acknowledge that the training, programs and events held by Sueno Fitness Inc. DBA G.A.C Fitness may expose me to many inherent risks, including accidents, injury, illness, or even death. I/We assume all risk of injuries associated with participation including, but not limited to, falls, contact with other participants, the effects of the weather, including high heat and/or humidity, and all other such risks being known and appreciated by me. I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in an activity. I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in. After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and furnishing services to me, I agree, for myself and anyone entitled to act on my behalf, to HOLD HARMLESS, WAIVE AND RELEASE G.A.C Fitness, its officers, agents, employees, organizers, representatives, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in Sueno Fitness Inc. DBA as G.A.C Fitness , programs and/or events. By my signature I/We indicate that I/We have read and understand this Waiver of Liability. IN CONSIDERATION of being permitted to participate in any way in any event (“Activity”) at any time during the current membership year I, for myself, my personal representatives, assigns, heirs, and next of kin: 1. ACKNOWLEDGE, agree, and represent that I understand the nature of the Activity and that I am qualified, in good health, and in proper physical condition to participate in such Activity. I further agree and warrant that if, at any time, I believe the conditions to be unsafe, I will immediately discontinue further participation in the Activity. 2. FULLY UNDERSTAND that: (a) THIS ACTIVITY INVOLVES RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH (“Risks”); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, the conditions in which the Activity takes place, or THE NEGLIGENCE OF THE “RELEASEES” NAMED BELOW; (c) there may be OTHER RISKS or SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation, or that of the minor, in the Activity. 3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE the sanctioning organization(s), their administrators, directors, agents, officers, members, volunteers, and employees, other participants, officials, rescue personnel, sponsors, advertisers, owners and lessees of Premises on which the Activity is conducted, (each of the forgoing shall be considered one of the RELEASEES herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED, OR ALLEGED TO BE CAUSED, IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releasees, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may be incurred as the result of such claim. (must be completed by Parent/Guardian for any participant under the age of 18) AND I, THE MINOR’S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF THE ACTIVITY AND THE MINOR’S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEES FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR’S ACCOUNT CAUSED, OR ALLEGED TO BE CAUSED, IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE “RELEASEES” OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS AND FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR’S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR ANY COST THAT MAY OCCUR AS A RESULT OF ANY SUCH CLAIM.

Release and Waiver of Liability and Indemnity Agreement
In consideration of being permitted to participate in any way in National United Wrestling Association for Youth (NUWAY) indicated below and/or being permitted to enter for any purpose any restricted area (here in defined as any area where in admittance to the general public is prohibited), the parent(s) and/or legal guardian(s) of the minor participant named below agree:1. The parent(s) and/or legal guardian(s) will instruct the minor participant that prior to participating in the below National United Wrestling Association for Youth (NUWAY) event, he or she should inspect the facilities and equipment to be used, and if he or she believes anything is unsafe, the participant should immediately advise the officials of such condition and refuse to participate. I understand and agreed that, if at any time, I feel anything to be UNSAFE; I will immediately take all precautions to avoid the unsafe area and REFUSE TO PARTICIPATE further.2. I/WE fully understands and acknowledges that:(a) There are risks and dangers associated with participation in National United Wrestling Association for Youth (NUWAY) events and activities, which could result in bodily injury partial and/or total disability, paralysis and death.(b) The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe.(c) These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releases named below.(d) There may be other risks not known or are not reasonably foreseeable at his time.3. I/WE accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis or death, however caused and whether caused in whole or in part by the negligence of the Releases named below.4. I/WE HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE National United Wrestling Association for Youth (NUWAY) facility used by the participant, including its owners, managers, promoters, lessees of premises used to conduct National United Wrestling Association for Youth (NUWAY) event or program, premises and event inspectors, underwriters, consultants and others who give recommendations, directions, or instructions to engage in risk evaluation or loss control activities regarding National United Wrestling Association for Youth (NUWAY) facility or events held at such facility and each of them, their directors, officers, agents, employees, all for the purposes herein referred to as �Releasee�...FROM ALL LIABILITY TO THE UNDERSIGNED, my/our personal representatives, assigns, executors, heirs and next to kin FOR ANY AND ALL CLAIMS, DEMANDS, LOSSES OR DAMAGES AND ANY CLAIMS OR DEMANDS THEREFORE ON ACCOUNT OF ANY INJURY, INCLUDING BUT NOT LIMITED TO THE DEATH OF THE PARTICIPANT OR DAMAGE TO PROPERTY, ARISING OUT OF OR RELATING TO THE EVENT(S) CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE RELEASEE OR OTHERWISE.5. I/WE HEREBY acknowledges that THE ACTIVITIES OF THE EVENT (S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage. Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES.6. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.7. On behalf of the participant and individually, the undersigned partner(s) and/or legal guardian(s) for the minor participant execute this Waiver and Release.
