Peer Network and Toolkit Survey
MadWorC is trying to formalize our network of peer support for cooperatives. We're asking as many co-op members as possible (whether your co-op is established or still developing) to fill out this survey so we can help assess our community's needs, individual needs, and figure out how we can match up people or co-ops who have what the other needs.

Please send this survey to everyone else in your co-op -- we are trying to reach as many worker-owners as possible.

This survey is only for people in the Madison, WI area. If you are a worker-owner outside of that area, we suggest reaching out to the Co-op Clinic at the US Federation of Worker Cooperatives.
