Please fill out this form for a birthday party request at OMT! Parties can be booked up to 3 months in advance and require a $50 deposit once booked. We will get back to you as soon as possible to approve your party date & time!

I agree to the timeslot previously stated and booked with On My Toes and understand that going over the hours previously booked may end up in being charged for additional time after 15-20 minutes of overtime.($25 for each additional 30 minutes) *
