5th Grade Party Liability Release Form: I hereby authorize that my child may participate in any/all activities, including being photographed during party events, provided by the WCE PTA at the 5th Grade Party, to be held at Lifetime Fitness 23211 Cinco Ranch Blvd Katy Tx, 77494 from 5:00 – 8:00 PM. I release and hold harmless the WCE PTA, it's officers, directors, members, agents, affiliates and WoodCreek Elementary School, it's administration, staff, employees, agents and affiliates for, from and against any and all liability, injury sustained, damage to or loss of personal property arising directly or indirectly while participating in any/all activities. *
