23-24 College Application Notification      Transcript Request Form
Please fill in the information below as completely as possible and submit
For each school do the following:
1. Once you submit your college application and pay the application fee, go to Naviance and be sure it is listed in "Colleges I'm applying to."
2. Check to see if your college requires letters of recommendations. Do not exceed the college’s number of recommendations. You must speak to the teacher before requesting in Naviance. Follow up with a thank-you note.
3. If applying with standardized tests scores, students are responsible for sending to the college via the test sites: ACT: www.act.org or SAT: www.collegeboard.org
4. If you have applied through the Common Application you must match your Common App with Naviance.
