SNOMED Request Form

To request a SNOMED code to be added to Accurx, please let us know the Concept ID (numerical code) and the Description of the code you'd like us to add.

We review and add new SNOMED codes on a monthly basis.

Please note, we are only able to add codes that are supported in both EMIS & SystmOne. Occasionally, some SNOMED codes are not supported in SystmOne. Where possible, we search for an alternative CT3V code, but if we are not able to find a suitable alternative, we will not be able to add the requested code.

Before you submit

To accurately submit your request please make sure to:

Request 1 SNOMED Code at a time in the form,
Restart the form to make additional requests.
