COVID-19 Home Asymptomatic Testing - Staff Results

Please complete the form below following completion of your home test.

If your test is NEGATIVE you can attend work as normal.

If your test is INVALID you will need to retake a test and resubmit the results.

If your test is POSITIVE (two lines on the LFT) you must have a PCR test at a Covid Testing Site to confirm the result. Tests should be booked via the Government website – https://www.gov.uk/get-coronavirus-test – or the NHS 119 dedicated helpline.

While you await a Covid Test (PCR) and the test results, all members of the household must self-isolate and remain at home.

Please contact your line-manager as soon as possible if your home test is positive.
