EFIT Session Information & Consent for Treatment
While we at All Is One Wellness Arts have extensive experience handling and caring for horses, we strongly suggest compliance with as many of the following session guidelines as possible.  These are meant to ensure the practitioner's safety as well as to provide the horse a safe, comfortable space where s/he can be at ease and open to treatment, and thus facilitating the best use of session time and the best outcome for your horse.

SESSION GUIDELINES:

Contact & Communication
_____Phone calls/texting during daylight hours only, can email anytime

Client Intake & Expectations
_____Electronic intake form must be completed before scheduling an appointment
_____A 50% deposit is requested to confirm your appointment
_____All Is One reserves the right to cancel or abort appointments if the horse or facility is a safety risk, or if in the opinion of the bodyworker the EFIT session is contraindicated.  
_____Appointments are 2 hours long and will not be extended if horse/client is not on time or not ready for the session to commence
_____Travel charges may be applicable ($1/minute travel time or $1/mile, whichever is less)

Expectations During the Appointment
_____Horse should be caught and groomed prior to the session.  If horse is dirty there will be a fee ($50)
_____No fly spray please (unless flies are a deterrent) and no show sheen
_____Owner, trainer, or responsible party must be present for the entire session
_____Facility must include a proper tie area such as a hitching rail (preferred) or cross ties.  NO work will be done in stalls, tied to a tree or fence etc.  
_____Facility must be clean (no muck or mud) or the client must provide a means for clean up/sanitization afterwards.
_____Tie area/work area must be clear of objects that the horse may run into if it starts moving around
_____Tie area/work area preferably is out of the way of other activities and away from other horses
_____Horse should be fed prior to appointment.  No work will be done while the horse is eating or if the horse is agitated about missing its mealtime. Please NO snacks during the session.

Payment
_____Payment of balance is due upon completion of services (at the end of the session).  All fees are + 4.166% HI GET

Cancellation Policy
_____Should you need to cancel or reschedule your appointment please notify me at least 24 hours in advance or you will be charged for the appointment.



CONSENT FOR TREATMENT:

_____ I understand that the EFIT session is for the purpose of releasing fascial constrictions in the body and across tissue chains to facilitate correct biomechanical movement, ease of movement, and release of pain. I understand very clearly that an EFIT session is not a substitute for medical diagnosis and treatment. EFIT practitioners do not diagnose conditions nor do they prescribe, perform medical treatment, nor interfere with the treatment of a licensed medical professional. I understand that is recommended I take my animal to a licensed veterinarian or licensed health-care professional for any ailment my animal has.  

_____The above specifications and conditions are satisfactory and are hereby accepted.  All Is One Wellness Arts, LLC has consent to provide therapeutic services for my animal.  Payments will be made as outlined above.  If you are not the owner you will be required to provide proof of owner consent.
