24-25 Annual School Dental Screening Parent Form

Dear Parents:

Rhode Island State Law section 16-21-9(b) mandates that all students in elementary schools be examined by a dentist at least ONCE a year, and then, at least once between the sixth (6th) and tenth (10th) grades.

Students who are seen yearly by their own dentist do not need to be screened at school.

Please indicate below ONLY if you would like your child to be screened by our school dentist this year.
