Set Up Crew (2 needed - arrive at 9:30 to set up chairs and worship space)

Assisting Minister

Acolyte

Crucifer

Altar Guild (1 needed - 1 chalice with wine, paten with large wafer, 1 purificator, missal stand, pre-packaged Communion in basket - refill altar candles, linen on altar - please have Communion ready by 9:15))

Usher (2 needed - gloved, assist with bulletins & communion, in charge of seating and entrance/exit flow - please arrive at 9:30)

Greeters (2 needed - give instructions, ask questions/take temperatures at the door, check worship reservations - please arrive at 9:15)

Cleaners (1-2 needed - wipe down/spray down chairs, high touch areas [doorknobs, bathroom sprayed with disinfectant after service], wipe down cross, acolyte torch)

LINK Driver (1st Sunday of the month - pack up and drive food to Herndon UMC)

By completing this form, I/we agree to the following (all of these are required): *

Masks are required to be worn by everyone (over age 2) at all times while in or around the building

Social distancing of 6 feet will be required at all times (families may sit together)

No socializing indoors before/after worship services. No hugs, handshakes, physical contact, etc.

Temperature checks and/or health questions may be asked prior to entry

Any individuals who are displaying signs or any symptoms of illness (or have been in contact with a confirmed or suspected case within the last 14 days) should NOT leave their homes and will not be able to enter the church

If you develop symptoms or test positive for COVID-19, let us know immediately so we can notify (maintaining confidentiality) the rest of the people who were present for that service.
