Former Student Transcript Request
If you are born before 1980, please contact the central registrar for the Sioux Falls School District: 605-367-4386.
All other former students have three options. 1) Stop by the counseling office at LHS and complete a green form and pay the $5 fee for each transcript ordered, or 2) print the PDF file listed on this website , fill it out, and mail it, along with the $5 fee for each transcript ordered to: Lincoln High School Registrar, 2900 S Cliff Ave, Sioux Falls, SD 57105, or 3) Fill out the Former Student Transcript Request form below and pay the $5 fee for each transcript ordered at LHS or mail the fee to the address indicated earlier. Transcripts must be paid for before they are mailed out. At this time, we are only able to accept cash, check, or money orders.
