Enrollment into the After-School Enrichment Program is achieved by completing the form below. The cost of the program is $55.00 per week with a one time $25.00 enrollment fee. Enrollment is only required for full-time attendance.
NO ENROLLMENT IS NECESSARY FOR DROP-IN ATTENDANCE. BY ENROLLING INTO ASEP YOU UNDERSTAND THAT YOU WILL BE BILLED ON A FULL-TIME BASIS, ACCORDING TO THE SCHEDULE OUTLINED AT
www.elcaonline.org/asep, WITHOUT REGARD TO ATTENDANCE. ONCE ENROLLED, A WITHDRAWAL FORM MUST BE COMPLETED TO CHANGE YOUR ASEP STATUS FROM FULL-TIME TO DROP-IN.
PLEASE COMPLETE A SEPARATE FORM FOR EACH STUDENT.
By completing this form, you agree that the weekly cost of ASEP will be added to your tuition statement as long as your child is enrolled, regardless of the number of days attended. If you wish to withdraw your child from the pro-gram, a withdrawal form must be completed to cease billing. There is a $25 fee to re-enroll after having with-drawn.
