Departmental specific questions for end of semester student feedback surveys
Please use this form to indicate the questions your department would like to add from the question pool to your department's end of semester student feedback survey. After each question you will need to indicate if it should be labeled an overall question or a people-based question and whether the question should apply to all courses within the department or specific types of courses based on their components.

Each department may choose up to 4 questions from the question pool to add to the common question set. Please enter each question separately using the form below. You will be asked a series of follow up questions about each question you've chosen. 

There are two types of questions that can be asked on end of semester student feedback surveys, general questions and people based questions. 

General questions are asked about the course and results of these questions will be visible to all instructors and TAs associated with the course. 

People based questions are asked about each instructor or TA associated with the course separately. Results of these questions are only visible to the instructor of which the question was asked and will not be visible to other instructors of the course if it is co-taught or to the TAs of the course.
