Please use this form if:

1) You have not received your initial Volunteer T-shirt or
2) You are a long term Volunteer and have not received a new shirt in the past two years

Shirts are awarded after 30 days of service and additional shirts are available for sale in our Spreadshirt Shop (https://shop.spreadshirt.com/sfas-volunteers/). Clothing items in the shop are considered official merchandise to wear during your shifts. Long-sleeves are required in some projects and these are printed with the Volunteer logo!
