This form is no longer accepting responses. If you still have a problem with your schedule, please come to Guidance and fill out a Schedule Change Form. Also, please keep in mind that:
 No change of lunch period is permitted without a note from the student’s doctor.
 Change of a teacher is not permitted.
 All students are strongly advised to carry a full program of at least five courses each semester in addition to PE. Counselors are not permitted to make changes that impact on this course-load.
 Students should follow the schedule they have been given until notified by the counselor that a change has been made.
