Uniting for Hope and Healing: A Benefit for Zoe Ward

If you are a duet or group, only one person needs to fill out and submit this form.  Please remember this is a benefit for a child with cancer and acts should reflect uplifting and inspirational tones.  Video submissions must be received on or before Thursday, July 27th.  Those selected will be notified by Thursday, August 3.  Note* you must be available for a dress rehearsal Thursday, August 10th at 5:30. Performance Friday, August 11th, at 7:00pm.

All acts are limited to 3 minutes. What kind of act (song, dance, comedy, acro, etc.) will you be performing? Acts must be appropriate for all ages. *
Are you auditioning for Mary Poppins which is the same night as the dress rehearsal? We will work around this but need to know ahead of time. *
If you are selected to perform there is a $20 performance fee for solo acts and a $40 performance fee for duets or groups.  (The performance fees are donated 100% to the Ward family.)  If selected, are you able to donate the performance fee? *
If you are not able to donate the performance fee, are you able to donate time before and after the performance for setup and cleanup? *
Lastly, please send a 30 second clip of the act you would like to perform. (Please label your file with your name). Email your file to whctonstage@gmail.com *
