RELEASE AND WAIVER OF LIABILITY I, the undersigned client, desire to participate or engage in activities related toservices provided by Waterview Wolves, LLC. These services include participation in physical fitness activities, personaltraining and the recording of and producing of videos and/or images. I freely, voluntarily and without duress agree to andexecute this Release and Waiver of Liability under the following terms: 1. ASSUMPTION OF RISK. I understand that theactivities in which I will participate in are inherently dangerous and can cause serious injury including bodily injury, damageto personal property and/or death. I acknowledge, agree and represent that I am qualified, in good health and in properphysical condition to participate in these activities, and that it is my responsibility to consult with a physician prior to andregarding my participation in any services offered by Waterview Wolves, LLC. I am voluntarily participating in these activitieswith knowledge of the hazards and potential dangers involved, and agree to assume and acceptancy and all risks of personalinjury, harm, and property damage. 2. RELEASE AND WAIVER. I release and forever discharge and hold harmless WaterviewWolves, LLC, and its officers, directors, employees, agents, successors, and assigns, from any and all liabilities, demands, orclaims that I, my heirs, agents, personal representatives, or assigns may have, whether active or passive, known or unknown,against Waterview Wolves, LLC, and its officers, directors, employees, agents, successors, and assigns, with respect to anybodily injury, personal injury, illness, death, or property damage that may result from or be caused by my participation inservices provided by Waterview Wolves, LLC. I UNDERSTAND, ACKNOWLEDGE, AND AGREE THAT THE LIABILITIES,DEMANDS, AND CLAIMS BEING RELEASED HEREIN INCLUDE, WITHOUT LIMITATION, LIABILITIES AND CLAIMS BASED ON ORIN ANY WAY RELATED TO THE NEGLIGENCE OR GROSS NEGLIGENCE OF Waterview Wolves, LLC, OR ITS OFFICERS,DIRECTORS, EMPLOYEES, OR AGENTS. I also understand that Waterview Wolves, LLC does not assume any responsibility foror obligation to provide financial assistance or other assistance in the event of injury or illness. I also release and foreverdischarge Waterview Wolves, LLC from any claims, damages or liabilities which arise or may hereinafter arise on account ofany first aid, treatment, or aid rendered in connection with my participation in services offered by Waterview Wolves, LLC. 3.MEDICAL TREATMENT. I authorize Waterview Wolves, LLC to seek emergency medical treatment on my behalf in case ofinjury, accident, or illness to me arising from my participation in services offered by Waterview Wolves, LLC. I understandand acknowledge that I will be responsible for any and all medical costs incurred by such accident, illness, or injury. 4.Waterview Wolves, LLC PROPERTY. I understand that the materials and tools provided by Waterview Wolves, LLC are andremain the property of Waterview Wolves, LLC, and I agree to return these tools and any remaining materials to WaterviewWolves, LLC at the end of my participation in any services provided. 5. PHOTOGRAPHIC RELEASE. I grant and convey toWaterview Wolves, LLC all right, title and interest in any and all photographic images and video or audio recordings made byWaterview Wolves, LLC, or its agents, during my participation in services offered by Waterview Wolves, LLC, including, but notlimited to, any royalties, proceeds, or other benefits derived from such photographs or recordings. 6. ACKNOWLEDGEMENT.I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. 7. OTHER. I expressly agree that thisRelease and Waiver of Liability is intended to be as broad and inclusive as permitted by the laws of the State of Texas andthat it shall be governed and interpreted in accordance with the laws of the State of Texas. Furthermore, I expressly agreethat if any one or more of the provisions contained in this Release and Waiver of Liability are held to be invalid, the invalidityof such clause or provision shall not otherwise affect the remaining provisions of this Waiver and Release of Liability, whichshall otherwise continue to be enforceable. If client is under 18 years of age or under a legal disability, then the parent orlegal guardian of the client must sign the following: As parent or legal guardian of the above named client, I hereby give myconsent for such client to participate in the services herein described, and by the signature below, acknowledge that I haveread, understand, join in and agree to be bound by the terms and conditions of the Release and Waiver of Liability. *
