T-Mobile Hotspot Request Form

If you have multiple students or students at multiple schools, please only fill out this form for one student. Enter your email address (not the student email address) when completing the form. You should complete the rest of the form with the student information you want the hotspot to be sent home with.

District 197 will provide Wifi at home through a T-Mobile Hotspot device for our students without reliable internet access at home. Both the T-Mobile HotSpot device and monthly fees are fully paid by District 197.

Hotspots are available to students enrolled at any one of our schools. The limit is one hotspot per household because each hotspot can support multiple devices.
