Release forms for each participant (creator or actor), with parental consent for those under 18, must be submitted by the deadline above for your entry to be considered complete and eligible for a prize (downloaded from contest web page).

All contest rules, as found at  must be followed and Fair Districts PA and theLeague of Women Voters of Pennsylvania retain the right to determine if the submitted entries comply with these rules.

Submission of this entry form indicates understanding of and agreement with contest rules and disclosures.

The participant understands that the use of the submitted video is completely at the discretion of Fair Districts PA and the League of Women Voters of Pennsylvania. Entries will not be returned. Fair Districts PA and the League of Women Voters of Pennsylvania will not make any payment to the participant or any third party for usage of the video.

I hereby grant to Fair Districts PA and the League of Women Voters of Pennsylvania nonexclusive and irrevocable permission to use, reproduce, prepare derivative works, distribute copies to the public, display publicly in any media, and by any manner and means, now known or hereafter devised, worldwide, for purposes consistent with FDPA and LWVPA missions. Additionally, I waive any claims for compensation for services rendered for production of the video described above.

I warrant and represent that the video I submit is my own original work, does not infringe any copyright, does not invade any right of privacy or publicity, does not contain any defamatory, libelous, or slanderous material, nor violates any other right of any person or entity, and is not otherwise contrary to law.

I hereby waive any right of attribution, right of integrity, moral rights, or right of inspection or approval of the use that may be made of the video, my name, and my comment(s) and information by IFDPA and LWVPA. I hereby release FDPA and LWVPA, their officers and employees, and those acting on their behalf, from liability for any violation of any right I may have in connection with the foregoing use.

I hereby give my full permission for the use of my name and videos, in whole or in part, individually or in conjunction with other images, as part of my videos submitted for the Gerry-Explainer Video Contest.

I waive all rights of privacy or compensation, which I may have in connection with such use of my name and videos.

I waive any right to inspect or approve the finished version(s), including written copy that may be created in connection with the video editing and promotion therewith.
