Chase 2022 Awards of Excellence

The Chase & District Chamber of Commerce invites you to participate in the 2022 Awards of Excellence to recognize individuals and businesses that have made a significant and lasting contribution to our community this year.

This form contains nomination fields for three categories, Business of the Year, Employee of the Year, and Citizen of the Year. You may make a nomination for any or all of the categories, though only one submission per category by nominator will be accepted.

Requirements for all categories:

  • No self nominations.

  • Chamber membership is not a requisite.

  • Nominations must include a short description on why/how the business or individual has demonstrated elements of the nomination considerations.

  • Current Chase & District Chamber of Commerce board of directors may not be nominated.

  • Nominations received without completed nominator information will not be considered.
