Revolution FC 2021-2022 Season Sign Up Registration
To register for Revolution FC competitive Soccer Tryout for the 2021-2022 season, please complete this form and submit by Noon, day of Tryout.

THIS FORM IS ONLY FOR TRYOUT REGISTRATION. BY FILLING OUT THIS FORM YOU ARE NOT YET COMMITING TO PLAY WITH REVOLUTION FC. OFFERS WILL BE MADE STARTING JUNE 13, AT THAT TIME YOU WILL SIGN YOUR CONTRACT TO COMMIT TO PLAY.

Players should bring their own ball and water to kick around. Please be prepared upon arrival to the fields by having all soccer cleats, socks, and shin guards on before taking the field. Please check website prior to date for any updates on time, place, dates.

By submitting this form I, the parent/guardian of the individual named below, do hereby agree to indemnify and hold Revolution FC, their coaches, staff and volunteers from and against all liability resulting from participation in competitive soccer try outs. I understand that soccer has inherent risk, for which I agree to assume complete liability. Furthermore, the player listed below is in good physical health for the activities which they will be participating. In the event that my child requires medical attention and I cannot be reached, I authorize Revolution FC to obtain medical treatment for my son/daughter with no liability to the cost incurred. I agree to pay all medical cost that incurred for my son/daughter or legal ward. Revolution FC is taking all precautions and guidelines set forth by both MYSA and our state and local health department to help protect all our players and their families from the spread of COVID-19. I understand the risk involved with allowing my child to participate. If my child or a member of our family contracts COVID-19, I do hereby agree to indemnify and hold Revolution FC, their coaches, staff and volunteers from and against all liability. *
