GSPL Math Club Consent Form
Please fill out this form to provide information for your child to attend the Frontier College/Greater Sudbury Public Library in-person Math Club for the 2022-2023 year. The program will run at the New Sudbury branch on Wednesday from 5:30pm to 6:30pm. The more information you provide, the more prepared we will be to accommodate your child. Please remember when registering your child that we require a commitment for your child(ren) to attend program every week, the same expectation that we have for our tutors.

Frontier College volunteers are subject to criminal record checks, reference checks, interview and training. Volunteers agree to comply with our zero-tolerance policies regarding appropriate conduct with learners. Please ask the Coordinator for more information regarding these policies (sudbury@frontiercollege.ca).

October 24 - program begins
December 2 - program ends for winter break
January 9 - program begins
February 21-25 - university reading week (no program)
March 13-17 - March Break (no program)
March 31 - program ends
