Media and Participation - Parental Consent

The POPDHH requests parental permission to use photographs and/or video recordings of your child or children participating in a school activity, and also the work they produce, such as creative writing and artwork. These photographs and/or videos may be used for external public viewing on the school or district website, in promotional materials and by news media for school and student related stories.

By completing this form, you consent to participation and use of media/artwork as described.
