TD POST Lottery Registration Form 2021-2022

PLEASE READ ENTIRE MESSAGE

Registrations will be accepted on a first come, first served basis where space is available. Please submit one form per child.
A registration will not be considered submitted until the fees are received.

Fees: $35 non-refundable registration fee per child,
           Non-refundable Security Deposit - amount dependent upon requested schedule.
  You will be asked to attest to the conditions of the security deposit before submitting this form.

All Before School care is provided at the Russell Street School TD site
After School care for Grades K, T & 1 is provided at the Shaker Lane School TD site
After School care for Grades 2-5 is provided at the Russell Street School TD site (2nd graders will be bused from SL to RS at school dismissal. Please be sure your child has a bus pass.)

Once submitted, the security deposit is nonrefundable.  The entire security deposit will be credited towards your child’s June, 2022 tuition, provided your child is still attending the same sessions.  If you leave the program or if you change your schedule, the security deposit will be forfeited.  The security deposit from one day cannot be transferred to another day. *
