Who is eligible to receive the COVID-19 vaccine?
Anyone 12 years of age and above is eligible to receive their first dose of the COVID-19 vaccine. Minors (persons between the age of 12-17) must be accompanied by a parent or legal guardian to receive the COVID-19 vaccine.

What do I bring to my appointment?
If you are receiving your FIRST DOSE of the COVID-19 vaccine, please bring your photo ID, appointment email (printed or digital) to your appointment.
If you are receiving your SECOND DOSE of the COVID-19 vaccine, please bring your photo ID, appointment email (printed or digital) and COVID-19 vaccination card to your appointment.
For individuals age 12-17, government-issued identification is required to validate age (passport, birth certificate, etc.)
Can I walk-in without making an appointment?
Walk-ins WILL be accepted while supplies last. In order to avoid unnecessary waiting, it is best for you to schedule an appointment.
Are there any special considerations in order for me to be eligible to receive the COVID-19 vaccine?
In order to receive the COVID-19 vaccine, you must meet the following pre-screening eligibility criteria:
Must not currently be infected with COVID-19, or previously exposed within the last 14 days.
Must not have received any other vaccine, such as the flu shot, in the last 14 days.
