PLEASE READ: This form will be the only way to submit a schedule change request for second semester of the 2021-22 school year. Counselors will take these requests on a first come, first serve basis. Requests cannot be guaranteed. Counselors will be in contact with you via email regarding your request, and may call you down directly to the office the first week back to school.
