If you would like to request the MassArt COVID Team set a recurring appointment for you at the same time each week, please provide your information below and we will do our best to book you a recurring appointment for the day/time you provide. A reminder that the testing site hours are Monday through Thursday 8:30am - 12pm, 1 - 4:30pm and Saturday 10am - 2pm.
