DVHS FERPA & Transcript Release Authorization form
All students applying to college must complete this FERPA Survey. DVHS will not send transcripts without this document on file.

Students and Parent(s)/Guardian(s), please complete this FERPA questionnaire/transcript release form. Students < 18 years old require a parent/guardian authorization. Students using the Common App to apply to colleges will complete this form on the Common App website as well as here.  

NOTE: Once you or your parent/guardian completes the survey, the response has to be processed.  Please allow up to 2 school days for processing before you can request transcripts.  Thank you.

1. IMPORTANT PRIVACY NOTE regarding FERPA: This is our own privacy waiver/FERPA and not a substitute for the Common App Privacy Waiver.  By signing this form, I authorize all schools I have attended to release all requested records covered under the Federal Educational Rights and Privacy Act (FERPA) so that my application may be reviewed by the institution(s) to which I am applying. I further authorize the admission officers reviewing my application; including seasonal staff employed for the sole purpose of evaluating applications, to contact officials at my current and former schools should they have questions about the school forms submitted on my behalf. ~~~ I waive my right to review all recommendations and supporting documents submitted by me or on my behalf to post-secondary institutions. I understand that my waiver or non-waiver selection above pertains to all colleges and career education programs to which I apply and that my selection cannot be changed once I invite my first recommender.

2. Release authorization includes: school transcript, secondary school report form, teacher report form, counselor report forms, and letters of recommendation. I understand that I am authorizing a release of such "official" school documents for the purpose of college admissions, service (military or volunteer), work/internship, scholarship applications, academic/athletic recruitment, and/or NCAA eligibility.
