This agreement limits our liability, please read it carefully. I agree to release Studio 5.6.7.8 Inc. its instructors, independent contractors and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family travelling to or from or during participation in activities and programs provided or sponsored by Studio 5.6.7.8 Inc.  I hereby acknowledge there are inherent risks, that may result in injury, associated with participating in physical activities including, but not limited to, dance, fitness or acrobatics that may include injury. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance, fitness or acrobatic class or other related physical activities.  I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability or even death.  I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including , but not limited to, employees, independent contractors, volunteers, and program participants and their families.  I release Studio 5.6.7.8 Inc., it's instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses known and unknown.  

Birthday party booking is not confirmed until payment is received. • Studio 5.6.7.8 Inc. accepts cash, cheque, direct withdraw, PayPal, or e-transfer to dance@studio5678.com • Party package A - $100 incl. HST  • Party Package B - $150.00 incl. HST. • Parties must be booked and paid at least 1 week in advance.  • NO REFUNDS for any reason, Studio credit only.  All credits remain with the Studio, have no cash value and are non-transferable.

COVID-19 has impacted us all.  Studio 5.6.7.8 Inc. is committed to providing a safe environment for our staff, dancers and families.  By agreeing to this policy you are helping promote health and safety in our facility. I agree:• To assess the wellness of my child or participant prior to arriving at the Studio.• To not come to the Studio if my child or participant is feeling unwell or exhibiting symptoms consistent with COVID-19 as described by local public health.• To not come to the Studio if I, my child, participant, caregiver or person transporting the participant has had close contact with someone confirmed to have COVID-19.  My child may return to the Studio once given clearance from public health.  • To allow my child or participant to be directed to sanitize their hands upon arrival.• To allow Studio 5.6.7.8 Inc. to refuse service to anyone suspected of being unwell or having COVID-19 until medical confirmation of a negative test can be demonstrated.• To allow Studio 5.6.7.8 Inc. to alter the delivery method of its services as required from time to time in accordance with public health recommendations. This may include but not limited to, online classes (live or pre-recorded), schedule modifications and/or class sizes.• To allow Studio 5.6.7.8 Inc. to limit entry to the facility to registered students and staff only as required from time to time.• To follow arrival, pickup, sanitation and physical distancing measures that may be in place and/or directed by Studio staff from time to time.• While every effort will be made to run all scheduled classes, no refunds or make-up classes will be given as a result of missed classes due to illness of the participant or teacher.• This policy may be revised as required to meet current public health guidelines.

Students/parents are not permitted to video or photograph classes, students or staff at anytime, unless given permission by the staff or individuals involved.• Photos of children should not be posted on social media platforms without the permission of the parent/guardian.• We may occasionally take photos and videos during classes and events, for promotional purposes. If you have any concerns about being in these photos, please advise us ahead of time. All photos taken by our instructors and administrators are the sole property of the Studio and will be used at our discretion.

Studio 5.6.7.8 Inc. keeps you informed using electronic communications. By agreeing to this policy you hereby grant Studio 5.6.7.8. Inc. express and exclusive permission to send SMS/text messages to your cell phone, emails to the address provided and messages to any social media contact method you provide.  You acknowledge that some of these messages may be commercial in nature and you are free to unsubscribe from this service at any time, by responding STOP to any text message you receive or clicking the opt out option for email communication within the parent portal.
