ARHE/ARS/AAPG Confidential Conference Issues Reporting Form
Welcome to the 2022 13th Annual ARHE/ARS/AAPG Conference Issues Reporting Form.

This form is designed so that conference attendees, staff/volunteers, presenters, and anyone else involved at our conference can have a direct reporting system for issues that come up, and to ensure that our conference is safe, equitable, and inclusive for everyone involved!

Issues may include, but are not limited to:
Technical Issues
Accessibility Issues/Concerns
Interpersonal Issues/Concerns
Discrimination/Harassment

Individuals may submit concerns anonymously, however all responses are confidential and can only be viewed by select conference staff/volunteers, as approved by the Conference Planning Committee, ARHE, ARS, and AAPG.

If you share your contact information we will try to follow up with you promptly. Response times may be longer during the actual conference itself. This form will be available during the conference, and will stay open for two weeks following the conference so that individuals may report issues/concerns at their convenience.
