2023 Banquet Registration
Please register each person separately. If you have more than 4 people, please fill out the form again. The cost for the banquet is $40 per person for adults and $15 for children 11 and under. If you would just like to attend the banquet without the dinner, the cost will be $10. You  may mail your payment to P.O. Box 393 Lebanon, OR 97355, pay using PayPal on the website, or pay at the door.
