Liability Waiver: I, the undersigned, understand that there are risks of physical injury due to the inherent activity of dance and its training. I hereby agree to release The Dance Conservatory, Inc, its officers, agents, employees, contractors, faculty, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as "The Dance Conservatory") and hold The Dance Conservatory harmless of all liability. I hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance for "Student / Caregiver" listed above, including any and all claims of negligence. I also give The Dance Conservatory permission to use photographs, videos, or any likeness of "Student / Caregiver" for purposes of advertising and marketing in any form for The Dance Conservatory or a The Dance Conservatory affiliated event.
