April - June 2021 Enrollment Commitment Form

Please complete this Enrollment Commitment Form to select a learning option for April 12 through the end of the year, which is June 4, 2021.

Please select the Full Remote Learning option if you do not want your child/children to attend school in-person.

Please select the In-Person Learning option if you want your child/children to enter the school building for in-person instruction.

Students in our half-day programs for EC, Pre-K, and Kindergarten will attend 4 days, (Mon, Tues, Thurs, Fri) per week either in the morning from 8:30-11:00, or in the afternoon from 12:15-2:45.

Students in grades 1-5 will attend 5 days per week from 8:30-2:10.

Students in grades 6-8 will attend 5 days per week from 8:20-2:00.

Specials and interventions will occur during the regular school day for both in-person and remote students.

Please complete a separate Enrollment Commitment Form for each child enrolled in any Worth School.
