I understand that by signing this form that I take responsibility for any injuries I/my child may receive and do not hold Xuan Sports and Wellness Pte. Ltd. responsible for any personal loss or injury incurred. I understand that it is my responsibility to inform the instructors of any medical condition that may affect my/my child’s health whilst under their instruction.
