HCC has federal grant from the Health and Human Resources Administration (HRSA) to train individuals in 2 college-level classes as Community Health Workers (CHWs), with a focus on behavioral health/addiction training, including opioid and other substance use disorders.

This form is NOT AN APPLICATION, rather a way to get on our program's radar. Once you complete the form, you will be sent next steps you need to take to apply and meet admissions requirements.
Under this funding you are making an 9 month commitment to completing 2 college level classes, 1 per semester. You are required to complete a 125 hour practicum placement as part of your 2nd and final class.
IF you are accepted into this grant funded program, your course tuition, fees and books will be provided for free.
In order to be eligible for the program, you must be eligible for College Level English, or Eng 101, at HCC in order to enroll in this program. In order to be eligible for the HRSA grant, you must be a US Citizen or US Permanent Resident.
You also must be able to go through the college's CORI/SORI process PRIOR to enrollment, in order to participate in this program. Please contact Kelly Lamas, Project Director with questions at

Students will also be required to be submit mandatory immunization records and complete a drug test in order to be eligible for the practicum class.**** Students who have not had a Hepatitis B shot should get started on this sequence PRIOR to starting their classes.*****
