Dear Parents / Guardians of Eagle Academy Students:

Thanks for allowing Eagle Academy PCS to serve your child (ren) during the
2024-2025 school year. Our goal is to have each of our current Eagle student’s join us back for the 2024-2025 scholastic year. An essential part of open enrollment for new students is having an accurate count of returning students. The information you provide will assist us in our planning to meet the needs of all our learners.

Reminder: The following documents are required for the Re-Enrollment each school year
• DC Proof of Residency Verification
• DC Universal Health Certificate, Immunizations (Due before the first day of school)

• DC Oral Health Form (Due before the first day of school)

All of the following forms and additional guidance is available on our school website under the enrollment tab. www.eagleacademypcs.org
