INTRADISTRICT TRANSFER POLICIES.  By checking the boxes below, you (the parent/guardian) agree to the following transfer policies:
1. I (the parent/guardian) will furnish transportation to and from the transfer school. *

2. My child must maintain satisfactory on-time daily attendance, citizenship/behavior and academic progress; otherwise, my child's transfer may be revoked at any time. *

6. I understand that my child will automatically be enrolled at my zoned middle school after 5th grade and zoned high school after 8th grade, which may result in my child attending middle or high school with different peers. *

I understand that: FOR OPEN ENROLLMENT REQUESTS : 1) I may not receive a response until the END OF MAY of 2021, which may result in selecting classes LATE, missing TRY-OUTS, enrolling late in BASES, etc., and 2) Status updates will NOT be available until the end of May.    FOR WAITLIST REQUESTS:  1) I will ONLY receive a response IF the request can be approved, which could be AFTER THE FIRST WEEK OF SCHOOL, and 2) Status updates will NOT be available for the Waitlist.  If I am not contacted by the 3rd week of school, it will mean that my request was not able to be approved from the Waitlist. *
