FOR VALUABLE CONSIDERATION, receipt of which is hereby acknowledged, I hereby grant the undersigned photographer (“Photographer”) the irrevocable right and permission, throughout the world, in connection with the photographs taken of me, or in which I may be included with others, the following: (a) the right to use and reuse, in any manner at all, said photographs, in whole or in part, modified or altered, either by themselves or in conjunction with other photographs, in any medium or form of distribution, and for any purposes whatsoever, including, without limitation, all promotional and advertising uses, and other trade purposes, as well as using my name in connection therewith, if he so desires; and (b) the right to copyright said photographs in his own name or in any other name that he may select. I waive the right to inspect or approve any use thereof. I hereby forever release and discharge Photographer from any and all claims, actions and demands arising out of or in connection with the use of said photographs, including, without limitation, any and all claims for invasion of privacy and libel. This release shall inure to the benefit of the assigns, licensees and legal representatives of Photographer, as well as the party(ies) for whom he took said photographs.

• I agree to attend all rehearsals for which I am scheduled*. I understand that it is extremely important that every actor be at rehearsal when called. For the good of the show and out of respect for the actors and staff, the director must know my part is covered. Attendance is taken at each rehearsal. Unexcused absences may result in having my role in the production reduced or in the case of extended absence; I may be dismissed from the production. *Parents may discuss conflicts outside of rehearsal times.


• I understand that arriving more than thirty minutes late to or leaving more than thirty minutes early, without prior approval from the director, constitutes an absence. 


• I understand that I was asked to list ALL conflicts on my conflict sheet BEFORE we begin rehearsals. If I am not present at any rehearsals during a time NOT on my conflict sheet, it will be considered an absence. If I have two unexcused absences and I am not already double cast or have an understudy, I may be removed from my assigned role. If I have three unexcused absences, I may be removed from the show entirely.

Please list rehearsal dates you are unable to attend. Camps are Mon-Fri, July 10-14, 17-21, 24-28, with one Saturday- July 15. Rehearsals for tech crew will be 10 am- 12 pm, and for performers it will be from 1-5 pm. **Tech Week is July 24-28, and everyone is expected to be there. Performances are July 28 & 29 at 7 pm, July 30 at 2 pm. Attendance at ALL performances is required. (If there are no conflicts, please type n/a)

Second Act Productions, LLC does its utmost to ensure the health and safety of its participants. Parents of minor children participating in SA productions are advised to be mindful of the fact that performance in (and rehearsing for) live theater has the potential for risk of danger (including, but not limited to, use of costumes, use of stage props, use of stage weapons such as blades or firearms, use of stage pyrotechnics, etc). 


I acknowledge that my participation in this production is voluntary. I acknowledge that I have been advised of the potential dangers of performance in live theater. After deliberation, I elect to voluntarily participate in this production. I agree to exercise due care and diligence while participating in this production, and to heed all instructions given to me with regard to safety and procedure in the rehearsal hall / performance venue and surrounding area. I agree to hold harmless (and waive any claim or action against) Second Act Productions, LLC, its agents, owners, directors, board of directors, producers and staff from any liability, fault or action arising from participation in this production.

Your commitment to Second Act Productions, LLC is to fully participate in our production(s). Should you decide not to participate, for any reason, you must notify us in writing within 48 hours of the cast list being posted on the Second Act Productions website. The performance fee to participate in this production is $550. The fee for tech crew is $275. (This includes your t-shirt, costume rental, and pre-performance meal on July 28.)

Unless an alternate payment plan has been arranged with Kelly Lepeska, all payments will be due by July 1, 2023. No fees will be refunded after this period.
