PARENTS/GUARDIANS:
Please complete one survey for each student that attended CCS in 2019-20, as the experiences may be different for each child. Parents/guardians may combine responses into one survey or fill out the survey separately. Where possible, please use primary CCS contact email addresses.

STUDENTS:
Students with a CCS email account are encouraged to complete this survey to reflect their personal responses.
Elementary age student input should be considered and incorporated into parent response.
