VHS Tuxedo Use Form

Please complete this form if you would like to use a VHS tuxedo set. We ask that you pay a use donation of $50 to help with maintenance and care of these uniforms. You may pay by check made out to the Ventura Music Boosters or by credit card via the link that will be given upon confirmation of completion of this form.

The tuxedo set consists of:

- Bow Tie (one size fits all)
- Cummerbund
- Tuxedo Shirt
- Tuxedo Jacket
- Tuxedo Pants

Please be sure to provide your size information for each item.

A sizing chart has been supplied for each item.

Each tuxedo set will require a bow tie - these are one size fits all, so no sizing information is requested on this form.
