Please note, this form should be completed by the exam candidate/student and not by the parent(s)

Applications should be made per paper/component. If you wish to request a post results service for more than one paper in the same subject a separate form (and fee) will be required for each paper.

Fees must be paid by Parentpay or BACS transfer (sort code 30-93-92 account no. 40736760) before submitting the form.
