Ballet Detroit 2019/2020 Uniform Order Form
All female students need to wear ballet pink tights, canvas flat shoes (Level 1 and up WITH RIBBONS), and pointe shoes if applicable. All male students need to wear a dance belt, white socks that come up to calf-level, and white canvas ballet shoes. Male students ages 11 and younger need to wear the full-seat dance belt and ages 12 and up need to wear the thong style. You are responsible for ordering these items separately.

Uniform Requirements for Each Level at Ballet Detroit:

Introduction to Dance, Introduction to Ballet, Pre-Ballet, and Primary Ballet - Lavender leotard with skirt
Level 1 - Pink camisole and 3/4-sleeve leotards, no skirt
Level 2 - Turquoise camisole and 3/4-sleeve leotards, no skirt
Level 3 - Perfect Plum camisole and 3/4-sleeve leotards, no skirt
Level 4 - Hunter Green camisole and 3/4-sleeve leotards, black skirt (to be worn as directed by instructors)
Level 5 - Navy camisole and 3/4-sleeve leotards, navy skirt (to be worn as directed by instructors)
All Male Students - white t-shirt and grey tights

ALL GIRLS MUST HAVE A MINIMUM OF 1 LEOTARD IN EACH STYLE (this means at least 1 camisole leotard AND 1 3/4-sleeve leotard). GIRLS IN LEVEL 4 AND 5 MUST ALSO HAVE A SKIRT.

Please submit your order form by January 15th. There is a late fee of $5/item if we receive your order after the 15th.

Introduction to Dance through Primary Ballet - $45
