Booking form for the after school activities at William Stukeley Primary school organised by Sport2day.  *Places are limited on each day and allocated on a first come first served basis* You will receive confirmation sent to the email you provide below and we will contact you should there be any problem with your booking. Multi Sports is a great way to try new things, we will have lots of sports each term with games like Football, Cricket, Tennis, Archery, Basketball, Netball, Golf, Ultimate Frisbee, Dodgeball, Rounders and many many more!

You can pay online via PayPal or Debit Card. Simply follow the instructions once you have submitted your booking. Don't pay if you are joining the waiting list for a full club, we will contact you should a spot open up and with details on how to pay for remaining sessions.

We occasionally take promotional photos/videos for marketing purposes. If you DO NOT wish your child to be included, please indicate here
Your child will need the following items with them for each day. Refillable water bottle, Hat/sunscreen or coat/jumper depending on weather and either school PE kit or sports clothing. For football club children should also have shin pads. Please tick the box to indicate they will bring them along on the day. We will collect your children at the end of the school day from their classroom and sign out from the main office gate at 4:20pm if you could please wait on the path/grass area. *
