Roslyn, NY Letter/Art Interest Form
Hey! Thank you so much for expressing interest. As you know, this project is all about helping the elders in our town during this pandemic. They will appreciate it more than you know, so thanks for helping out. You can either write a letter to a resident, or create any sort of artwork to give to them.

Please wait until a team member has reached out to you saying that you can start making your letter/art. This is because some facilities are giving us information about the residents we are writing to. We may be able to provide you with a name and interests of someone, which will make the letter more personal and meaningful.

Information about how to claim service hours, how to properly submit letters, letter/art ideas, and more can be found in this drive: https://drive.google.com/drive/folders/1Ehgrw2ytlnIoL1UN1CuGiQKypvg8P2-8?usp=sharing

So just fill this out if you decide to write a letter or make art so that we can give you the name of a resident! If you have any other questions, just email roslynlettersforrose@gmail.com

Name *
Phone Number and/or Email *
Where are you from? (City, State) *
How many letters/art do you want to make? *
Would you like to drop off your letter/art, have it picked up, or submit it digitally (drop off is encouraged, then pick up, and digital is if you live too far for either). *
For drop off- here are the locations we are collecting letters/art. You can choose whichever one is most convenient for you. *
For pick up- what is your address? (If you are not doing pick-up, just write n/a) *
For virtual (letting people handwrite your note is encouraged because they are more personal and meaningful to elders!) *
Are you interested in donating anything to the nursing homes (books, games, art supplies, diabetic socks...) *
Can you send this link to other friends you think might be interested? *
Thank you so much for taking time out of your day to do this! Make sure to mark your letter/artwork with your name (if you don't want to write it directly on it, you can attach a post it note with your name on it when dropping it off). This is to ensure we have collected all of the letters and we know whose we are missing. We highly suggest the letter is in an unsealed envelope. (Please keep in mind that there can be nothing about religion!)
We will reach out to you through email/text for more details and information :) questions can be emailed too roslynlettersforrose@gmail.com
