If you are not a registered member of Aldwinians RUFC you must complete this form before you can take part in any rugby related activity.
Rugby is a physical contact sport and there are inherent risks associated with participating in rugby related activities. All our coaches are qualified and recognise what is acceptable for all players in relation to their level of experience, but accidents can happen and may happen to you.
The purpose of this form is to collect data to use in case of an emergency.
