In consideration for my participation in Cameron Park Family Taekwondo Academy martial arts training, CPFTKDA training videos, CPFTKDA online training course, and/or use of any training gear or sparring gear purchased from CPFTKDA. I the student / parent / participate acknowledge the existence of certain inherent physical and health risks in this type of training and hereby agree to assume all risks when training, following the online video instruction, online training course, and/or using any training or sparring gear. I further relieve Cameron Park Family Taekwondo Academy, affiliated Martial Arts schools, it’s management, instructors, assigned staff, and fellow students from any liability resulting from personal injury or loss of belongings. I also hereby state that the students named above are physically fit to take the prescribed course of instruction and do so of their own free will. I understand there is a no refund policy on any monies I will pay Cameron Park Family Taekwondo Academy. I grant CPFTKDA permission to use all pictures and videos taken of classes/events for any legal use, including but not limited to; publications, copyright purposes, illustrations, advertising, and web content. I understand that no royalty or other compensation shall be payable to me by reason of such use. All Live Streaming media that permits recording may be recorded.
