This form may only be submitted by a club director or club administrator, not by a player or parent.  

The CEVA Region has the ability to grant waivers for boys to participate on junior girls’ teams.

Waivers are only valid within the Columbia Empire Region, at CEVA-sanctioned events.  Teams with a waivered player who travel outside the region must notify the tournament director and seek approval for the player to participate.  Boys are not allowed to compete in the USA Volleyball Girls Junior National Championships or Qualifiers, and may not participate in the CEVA Bid Tournament.  Guidelines for the waivers include, but are not limited to, the following:

1) Applications must be completed by the player, parent/guardian, and the club director, and must be approved by the CEVA Executive Director.

2) Each team may have no more than two male players on the roster.

3) Players must be defined by USAV as a 14-year-old or below.  Please see the Junior Girls' Handbook for a complete listing of teams boys are eligible to play on based on their age definitions.  

4) Waivers may be revoked should any part of the application be falsified or deemed misleading.  In case of revocation due to false/misleading information, the player and club director are subject to sanction, up to a season-long suspension.

5) CEVA has the right to revoke a waiver based on reports of safety concerns or other extenuating circumstances.  Revocation would only be done after consultation with the Junior Girls’ Committee.

6) BOGT waivers will not be considered for athletes who have anything other than a full junior membership.  Tryout memberships, Fall memberships, and single-event memberships must be upgraded prior to consideration.

7) Exceptional circumstances may be considered by the CEVA Executive Director.  

Waivers must be submitted by the club director to the CEVA office no later than 3:00 PM on the Wednesday prior to the tournament the boy wishes to participate in.  Player will be considered a part of that roster for the remainder of the season.  There is no limitation on how a boy may participate during a match if this waiver is granted.  Boys may play front row and are not limited to specific positions.

The application must be completed in its entirety.  Incomplete applications are not considered.  If this waiver is granted, the club coach will receive a copy and must carry it at all times during all CEVA-sanctioned competition.  CEVA can approve the player on the appropriate SportsEngine roster, but club is responsible for ensuring player appears on rosters in AES or other tournament management programs.

Please allow 48-72 hours for a decision on each waiver from the CEVA office, during a regular work week (does not include holidays or weekends).
