Dear parents, please help child fill up all information required for class registration.

Each child needs a separate registration form.

Both registration and payment prefer to be completed before 1st class started date. All supplies and tools are included.

Classes Fees:

December,2002 (3 Classes) $60.

Payment options: Zelle, Credit Card, PayPal Account* or Check.

*Pay with PayPal account will not charge transaction fee.

Seat Limitation: 24 seats/month classes.

Class location: Stem lab.

Kinder to class: Will be picked up from classroom.

Grade 1-4 to class: Will meet at Rondo Lobby bench area.

Child pick up location: Rondo school front entrance next to the admission office.

By registering, you agree: 1.Students are responsible for arriving on time. 2.Parent/Student agree to hold instructor and assistant of class, or employees harmless from any act of ordinary negligence, arising from any accident, injury, or damage whatsoever, however caused, to any persons or the property of any person or persons, or corporations occurring during all the times the student is participating in the class up to and including the time when the student is released into the responsible party's care. 3. We reserve the right to implement a $15.00 charge for every 15 minutes at the responsible party is late picking up the registered student. We also reserve the right to withdraw any student from class without refund, due to tardy pick-ups and/or misbehavior. No refund will be issued after the first class. 4.This class is not affiliated with ,or endorsed by the school or the school district. This material was not produced at district expense. 5. We reserve other rights including: a. Assign alternative instructor, assistants for classes b. cancel or change class schedule due to unpredictable weather, school's schedule change or potential safety concern.
